INTRODUCTION This Privacy Notice (“Privacy Notice”) describes the Personal Information that Evolus Australia Pty Ltd’s (ABN 76 663 925 166) having its registered address at Suite 2, Level 25, 100 Miller Street, North Sydney NSW 2060 (hereinafter, “Evolus”, “us” or “we”) collects and how we manage it.

PERSONAL INFORMATION AND HOW WE USE IT “Personal Information” is information or an opinion about an identified individual, or an individual who is reasonably identifiable. We will collect Personal Information whenever we interact with you in any manner. We collect Personal Information in a number of ways including, via our website, by telephone, and by e-mail. We also collect Personal Information from third parties including specialist data companies in our sector, publicly available resources of information, and marketing partners.
We collect your personal information so that we can: (a) provide you with products and services, and manage our relationship with you; (b) contact you, for example, to respond to your queries or complaints, or if we need to tell you something important; (c) comply with our legal obligations and assist government and law enforcement agencies or regulators; or (d) identify and tell you about other products or services that we think may be of interest to you.   • To specialist data companies in the healthcare sector involved in maintaining the accuracy of such data.
  • To other companies with which we collaborate in co-development, dissemination and/or marketing of products or services.
  • To comply with regulatory or legal requirements.
  • where we are otherwise permitted by law to disclose the information.
  • To any third-party legal successor in connection with preparing, completing, and implementing any sale of some or all of our assets, a merger, or reorganization, including by means of bankruptcy, which may occur.

DISCLOSURES OF TRANSFER OF VALUE An Evolus company in the country of your professional practice may make public disclosures of the transfers of value that you receive from any Evolus company in accordance with the EFPIA Transparency Code of Conduct and/or applicable local law.
